Dear Parent/Carer/Legal Guardian,
Re: Year Six online Health Questionnaire 2023/24.
The Department of Health recommends that children in Year six receive a health review by the School Nursing Team.
In order to achieve this each child in Year 6 will be provided with an online questionnaire in school about their physical health and emotional wellbeing. This provides an opportunity for children to think about their general health and wellbeing and for the School Nursing Team to address any issues identified. This questionnaire also aims to provide general information in the form of a personalised health care plan.
In order for your child to receive their health care plan they will need an email account. If they do not already have one, please can we ask you to support them by opening up an email account.
After the questionnaire has been completed, the School Nursing Team may see your child in school, simply to clarify the answers they have given to the questions or if they request it.
If we see your child we will give them a letter to inform you of this. We may need to also contact you to verify and add to the information given. We encourage you to speak with your child about the outcome of their questionnaire and discuss their personalised health plan with them.
The personal information collected from this questionnaire will be treated as confidential. Anonymous and collective information may be shared with our partner agencies to improve services to children. If you wish to contact the School Nursing team to discuss this further please telephone the above number.
You and your child might like to access www.healthforkids.co.uk which will provide information and support about children’s physical and emotional health.
If you do not want your child to participate, please contact your child’s school to make them aware. You will need to do this within 5 working days from receiving this letter.
